The purpose of this program is to identify individuals who pose a risk to themselves and their co-workers as reflected by there safety performance record .The implementation of this program utilizes safety training, peer support, and (eventually) disciplinary action to encourage and foster a positive change concerning unsafe work habits.
1. The following point value system will be utilized for assessment purposes for each individual accident. (Occupational illnesses are not addressed in this program).
a. Lost Time =12 points
B.Restricted Duty = 8 points
C.MSHA / State Reportable = 4 points
D.First Aid Only = 0 points
E.File Only= 0 points
f. Poor job safety observation = 4 points
2. The following process will be utilized to identify AT RISK WORKERS:
Step 1: When an employee accumulates a total of (16 points) the safety director will notify the employee in writing as to the express concern about the number of accidents that person has experienced. Upon receipt of this notification the employee will now be classified as an AT RISK WORKER.
Step 2. The employee will receive paid training and peer counseling that will entail but is not limited to the following, accident review, accident prevention, total cost review concerning his accident & work hardening program etc.
Step 3.If after a six month period of work the employee has had no other accident or poor job safety observation he will no longer be classified as an At Risk Worker.
Note: All AT RISK WORKERS WILL HAVE TWO JOB SAFETY OBSERVATIONS CONDUCTED PER MONTH.
Step 4.In the event that the person accumulates 20 points or more within a 12 month period of work disciplinary action will be initiated so that person and his coworkers will remain safe and unharmed.
In the event an employee does not wish to participate completely with all facets of the afore mentioned program they will be subject to disciplinary actions up to and including discharge.
